1. Close apps and right click on your desktop.
2. Choose Nvidia Control Panel
3. (Left Side) Choose Manage 3D Settings
4. Set Global Settings to High-Performance Nvidia Processor (see picture)
5. Apply
6. Launch Diablo 3 and see if your FPS improves.
7. Change back to Automatically Detect or Integrated Graphics when done with Diablo.

or you could just whitelist the actual app in the program settings tab and not stuff around with this every time?
just a thought -
If your using any driver before 304.48 you want to try the above. Whitelisting doesn't make a difference for some reason. Just try it. Again this applies for all drivers pre 304.48.

I thought it was actually quite a valid comment. With the same token, why not just right-click on the D3 icon and select it to run off the Nvidia high performance GPU instead of going into the Nvidia CP every time?

I'm running Pre-304.48 and it's smooth as butter on max settings, if while-listing doesn't fix your problems then you have other issues \ drivers etc, remember the latest isn't always the greatest mate
what you are actually doing is making more work for yourself by doing this method everytime. -
There is a reason to change the graphics processor to dedicated based on how the earlier drivers handle the game. Thus the method above is used even when Run As is an option. MANY GAMES IN THE PAST REQUIRED THIS! Whitelisting does not always make the game run correctly
Example of a game that did NOT work with whitelisting or RUN AS is Starcraft 2. When it first came out, it didn't fully utilize the GPU without changing the dedicated GPU in the nvidia control panel.
