You've been wonderful help so I thought I'd continue to ask for your support.
I've just upgraded my 6870 to a 6990, and now the fan is always on max by the sound of it - I thought the fan was only always on max on a 580M?
Is this normal? Can this be adjusted?

Stating that the fan turns on maximum speed with the 580M is completely false, who told you that?
As for your issue:
Did you install the 6990 yourself?
Did you or the person who install it properly applied thermal paste?!
How are your temperatures? -

however, you can download HWINFO which has a fan control on it. Try it. -
Did you put pads and paste on the GPU when you swapped it?

my fan only goes to full speed once it hits 70c 10 secs after its back at 60 lol

Yeah, the fan is thermally controlled (unless you are using HWINFO to override that). It will only spin to high speed if your temps get above 70C or so. At idle, temps should be low enough for the fan to be off (or at least silent).
You need to check your temps, since you may be damaging your card. -
cheers guys, I put the pads on thermal paste and actually temps and fan speed seem fine, but performance is terrible for some unknown reason... Do you know what the issue is?
I notice I can no longer switch to integrated graphics either...? -

ah, well what do I do? Uninstall the Dell driver? I already have 11.9 Catalyst installed for the 6990M...?

Ok, issue resolved. Mahoosive performance boost when I updated the driver to Rage enabling driver...
Chronicles of Riddick no longer loads, but that must be a single game issue as everything else is perfect! -
