It can be a faulty card or faulty software. I would first recommend doing a clean install of the 13.1 drivers and if it still persist, give Dell a call because you card is near its ends...

Driver Sweeper still leaves fragments, but a registry cleaner used after it will clean the remaining AMD fragments. Start by uninstalling the Catalyst install manager, then Driver sweeper, and then a registry cleaner, run the deep scan, it will grab all fragments left behind. If you have no idea what I am talking about/ or don't feel comfortable then just stick with the first step, and then reinstall. You can also do it in Safe Mode, sometimes the system will not remove a portion of a registry key due to it being utilized, so safe mode will be the best option IMO. if you already have the AMD driver of your choosing saved to your computer then to avoid your OS from trying to automatically DL the correct drivers make sure your Internet is not connected, this might not happen, but better to be safe.

It's Firefox only issue. I'm pretty sure that you can fix it by go to option - advanced - uncheck use hardware acceleration.
