The matrix has taken root in your computer...

I'm really bad with hardware but those are definitely screen artifacts. I don't know if they're RAM artifacts or BUS artifacts but the most common cause other than a faulty card is overheating and overclocking.
Where did you get the 580M and did you install it yourself?
If so, have you tried reseating the card?
Do you have the correct heatsink for the 580M?
Did you properly apply thermal paste and properly position all the thermal pads?
Is your GPU fan spinning?
Have you overclocked the card at all? -
That just means your backplate is not properly insulated

did yo disable the internal GPU (HD3000) in bios ? If not, then these artefacts is coming from HD3000, or ?

Thanks guys for reply

To restate Widezu69's query, does your 580M include the large thermal pad on the back of the card that serves to dissipate heat from one bank of VRAM and to insulate the card from the motherboard?

In the case of the copper portion of the heatsink not covering the die, I believe there is black tape that can be removed to expose more copper. Again, someone please verify. -

But one possible culprit in the artifacts problem is electrical interference caused by contact between the exposed circuits on the back of the 580M and the motherboard. A large thermal pad should be attached to the back as shown below; if you have one that looks and is positioned substantially like that one, then it's a false trail, and the problem lies elsewhere. If that pad is not there, that is very likely the problem.
Edit: @aznpos531 -- didn't see your post until after I had posted. We're on the same wavelength, but you have the better picture. -
Heh, I wasn't sure if I was totally accurate so it's good that you posted to confirm

thank you for your time to reply, so what I did is removed all the black material so the entire solid copper faced directly m580 die, re-applied thermal paste. I looked at the back of 580m , it had a large pad thermal attached on the back exactly like the one you had in the picture, put all back together, fired it up and the pictures was same as before nothing change.

I've had this issue before. I just added more sticker to the side of the backplate which makes contact with the GPU, the sticky bit. Solved it. You should try it with a bit of insulating tape.
