GTAIV is poorly optimized, I'll vouch for that. However, I played through the game with the setup in my signature on moderately high settings at 1920x1200. I could not run it with everything maxed out. You have the latest version of the game. Do you have the most updated drivers for your GPU? Make sure you install all of the CAP as well. With your GPU and CPU, you should be having a pretty enjoyable GTAIV experience. Its worth it to get your problems figured out. That is an incredible game.

i tried to run it with my 460m..... and lets just say I gave up on it. Tried hacks, mods, tweaks, and it still wouldn't see my 460, just intergrated.

Can't say I have any of the issues you describe when I play it. The major thing to check though would be that your GPU hasnt downclocked and then locked itself onto those idiot 400/900 clocks which it loves to do! Then you will have performance issues.

I am currently playing EFLC, and it runs at about 55 fps all of the time on medium/high settings with my single 5870M, at native res. My detail distance, vehicle density, and that other setting are all at 20.

The real nut-kickers are the shadows and the reflection quality. If you set everything Medium, completely kill all shadows (this sucks, I know), and just set all of your sliders to ~20, you should see a pretty good improvement in fps and smoothness. No, it won't look as good. Can't have your cake and eat it too. ;)
