    I have no idea am i using ati 6970 or 6990m?
    from device manager i could only see AMD 6900 hmm
    is there any way to recognize the exact model of my gpu?

    That is a 6990m. It has 1120 shaders and a clock of 715/900. You're all set to go :)

    may i know whats the max overclocking for this gpu mate?

    I dunno, 800/1000 is a good start. every card is different.

    I tried to run 850/1050 at stock voltage playing gta 4
    stable, first 5 seconds after loaded up my saved game, I could reach over 100fps :p
    Does this system has any throttling issue?

    Depends on the bios your using. That will determine throttling issues. A05 and above all have heat throttling.

    850 is really high. Start at 800 and work your way up slowly in 5 MHz steps. My highest stable gaming clock is 825 or 830 I think, but it will be different for everyone.

    As for the memory speed, you should be able to set it between 1150 and 1200 without issue. Just adjust only the core or only the memory at each increase (not both), so you can tell which is causing the problem.

    Also, this goes without saying, monitor your temps closely. Try to keep the GPU under 80C, or much lower if possible. I would suggest repasting/repadding prior to any attempts to find the limits of your overclock.
