Hey guys i just wanted to share a fix I found.
285.62 optimus problems - NVIDIA Forums
Post #12 was the winner for me. Past few days I have been having this problem where after upgrading to 285.62 drivers of nvidia and onwards, a lot of my media players were not giving me the option to let them use the nvidia GPU, the option was greyed out, and hence my Media Player Classic - Home Cinema could only render from the Intel GPU. the new beta ones didnt fix it either.
Well I applied the fix mentioned of just renaming the .exe of the media player to something else (i named mine exactly what the poster suggested) and behold, it worked! I was given the option to enable the GPU on the optimus menu, GPU turns on and is being used by media player classic (all video codecs and renderer use nvidia gpu now). I remember few ppl posted they were having the same issue so hopefully this helps out more ppl since i was going crazy seeking info.
Another fix of course was downgrading the drivers to 280.ish something but I didnt want to downgrade. :p

Does the newest driver fix the tearing-image problem when watching mkv/x264 video?

What screen tearing issue? Do you have Aero on or off (Keeping Aero enabled should rid you of screen tearing in general applications), and what program are you using to watch the videos?

At 267.76, using CCCP or Klite with nvidia enabled, I've screen tearing with most of my video.
Updating to 285.62, using CCCP with no nvidia, well, sometime sound and video are not sync well, it lag. 1080p is more horrible. With no nvidia but DXVA enable, syncing is better, but now subtitles are all messed-up.

Edit; enable aero fix screen tearing issue, hahaah, cant believe it works lol
    THanks @namaiki
