Material degradation caused by temperature variations, slight differences in motor balancing, particles inside the drive and moving the drive while in operation.

Reduce movement where possible while in use and keep it away from environmental extremes where possible.

To be fair you should not worry to much, just don't shake the machine or throw it about while it is on and don't leave it in your car on a hot summers day. If you use common sense unless you got a bad drive it should be fine.

Usually it is impact shock or pure luck. Most if not all modern machine have some kind of accelerometer based software to stop the hard drive when needed.

Of course these only apply to hard drives, SSDs are much more resilient to shock :)

Does improper shutdown also causes bad sectors?

Yes it can cause problems... However, sometimes you have no choice if your computer completely hangs etc so you do have to remove the power cable/battery etc and do such a shutdown!

Modern drives should not crash the head into the platter on power loss but do try to avoid it. It's more likely to corrupt data than physically damage the drive.
