Does it have a significant impact on battery life? Performance?

If you change settings between "maximize performance" and "maximize battery life" you will notice an extremely significant impact on battery life and performance (while playing games).

I haven't tried unchecking the checkmark.

That's debatable really, if you leave it on the max battery option then you will get better battery life but if you put it on max performance then the core clock rises from what it would be without powerplay enabled giving more likely worse battery life but better performance than you would have had if you hadn't enabled it.
