I have the Direct-2-drive version of the game. Will a retail patch work with my game?? Because D2D does not have any patches for GTA4 except the older ones.

Yep, it is always worth letting them know about any problems and it is great to see thet they have released a patch. I have been playing updated to this patch for a while now and I notice much smoother overall performance and using Vsync no longer causes worse performance for me. My Realizm 6.2 Modded install along with all of my other Mods is also still working perfectly!
This is likely to remain my favourite game for quite some time.
Edit: I found using the official patch to update my Realizm IV install directly worked the best. Doing it the usual way introduced graphical glitches and crashes. -
I had that water slowdown issue with the boat in the "Sexy Time" mission. I thought it was strange since otherwise, the game is buttery smooth on my rig.
I'll wait to apply the patch as it'll probably break the trainer I have for the game right now, makes it a whole lot more fun!

Your trainer, depending on type will probably still work. It will just need the updated Scripthook.dll or ScripthookDotnet.

I would like to update to this patch, but unfortunately the Alice modding script only works up to patch 1.0.4, and modding just makes the game more fun.

What FPS where you getting before? That looks like it runs really smooth.

I would get similar fps but the main difference was that I had to set Night shadows to medium or disabled in order to reduce stuttering. I had a look through some old GTA IV benchmarks and I was getting up to 55 fps and for the most, I had been using Dell Mobile Drivers 196.86 so I will give those a test again. Of course, in game my fps is mostly 40-45 fps and only occasionally hitting 50's and sometimes as low as 30 fps but it is very enjoyable.
