Basically he can only play games that use the mouse to move, and left/right click.
He has played Age of Empires III and all its expansions and loved it, so I am looking for recommendations for other games that require no to very minimal keyboard entry (ex: AoE III needed a keyboard to enter his name for the player profile and the 'Esc' key to skip past intro movies, and someone sitting beside him can do those simple key entries for him, as he cannot use a keyboard at all).
He has played Starcraft in the past and enjoyed it as well.
Any recommendations/suggestions?
For reference, the desktop he uses has a P4 2.0ghz, 1gb RAM, Radeon 9550 PCI 128MB, and 19" LCD (Its a Dell Dimension 2450 I believe).
He also has a new Latitude D630 with C2D 2.0ghz and integrated graphics, so it can play older, non-graphic intense games, which are just as good, as long as they're ufn.

If you have a PS2, you can get a couple of voice-based games:
- Karaoke Revolution series
- Lifeline
There are multiple companies working on mental-activated games... so far they have been successful with older games with basic mental triggers (attaching small headgear to detect brain activity):
- Space Invaders
- Super Mario -
What kinds/genres of games does he like? You mentioned RTS, does he like turn based stuff?---In general, turn based might be more suitable as in doesn't require fast reflexes.
Anything turn based from Chess to The Operational Art of War 3 can be played via mouse and doesn't depend on reflex speed. -
He liked the RTS style, he has been looking for a very basic shooting/sniping game, but really no luck so far. Found a basic flash or java based one on a web-based games site that he liked for a little while, but it really isnt much compared to a retail game, even one from 5 or 6 years ago.
I let him try Bejeweled 2 and he didnt like that style of game.
I'm just looking for suggestions here, and any good ones I'll load onto his PC and let him try out, hopefully there will be one or two that he likes. -
Starcraft. It's more than 10 years old and I still play it.

Command and Conquer 3: Tiberium Wars is a great game. Maybe he'll enjoy that.

I second Civilization 4. It's super deep, can be played with just a mouse, and is turn based, so you don't have to worry about having super quick motions or reflexes. I assume the quadriplegic mouse isn't very precise ($750, DAMN.)
Company of Heroes and Opposing Fronts (expack) can be played without a keyboard afaik. It's really a great game, one of my favorites. There are cutscenes that you would have to skip with the esc key but they're really top notch and generally short, so that shouldn't be a problem. Disadvantage is relatively high graphics requirements.
Have you considered an on screen keyboard for things like entering a name and such? Wouldn't be ideal for everything, but would allow for basic key-entering needs.
Red Alert II and Yuri's Revenge (expack) also come to mind. Both amazing games, old, but with stylized graphics that imo still look great. Both have amazing campaigns and don't require a keyboard (or even right clicks.) Not to mention you can probably find digital versions or bargain bin versions for $10.
Summery: Red Alert II and Civilization 4 are great games with just a mouse and/or limited twitch abilities. -
Anyways, your tax dollars buy these mice for quadraplegics, so dont sweat it. -

how does he like rpgs? knights of the old republic is a fun game, and you can pause the action sequences so that you can input specific actions.
if he likes games with good story lines, he could consider adventure games like grim fandango. -
How does he feel about turn-based strategy games? Civilization 4 was already mentioned (actually most of the Civilization series would be good, also Alpha Centauri etc.), but another suggestion is the Heroes of Might and Magic series. Out of five games, the first 3 should run well on integrated graphics. Strong TBS, good campaigns, story, graphics, etc.
Also, how about adventure type games? I'm fairly certain the first three games in the Myst series don't require a keyboard (at least Myst and Riven don't, not 100% on Myst 3: Exile but I don't think it does). Another strong suggestion would be Syberia I and II, both very good adventure games (I *believe* the keyboard may be required to hit Esc, however, in order to access the save/quit/etc. menus). -
Ah yes there are lots of old school games, still very fun today that use only a mouse. Lucas Arts adventure games to name a few

Indian Jones
Dune (awesome game, not to be confused with the rts Dune 2)
I actually enjoyed the RTS Krush Kill N Destroy, might be a goer.
Maybe even The Sims.
Mad Dog Mcree - This is a rail shooter using FMV. I really enjoyed this, it may be too fast though. Hopefully theres an easy play level.
I vaguely remember another game like this featuring FMV except your a cop and one of the missions is going into a bar. I only played the demo but it was far. Its all point and click too. -
Sins of a Solar Empire! The game is smart enough to manage a bunch of stuff without you. It completely rids players of the need for micromanagement if you get the AI working.
I think it could be a great candidate for your friend. -
The Battle for Middle Earth II, plus the rise of the witch king expansion.
Dawn of War, Dark Crusade, or any of the Dawn of War series. -
Well if he is accurate with the mouse, he could potentially try Diablo 2, everything in the game can be operated with left/right click and moving the mouse.

It's a little hard however possible to play World of Warcraft with just the mouse.

Warcraft III + expansion (Frozen Throne)
Diablo perhaps.
Diablo 2 but I haven't played it in a while, i'm pretty sure you can point and click through it.
Rise of nations is an excellent RTS.
Titan Quest is also similar to diablo/d2; its mostly point and click.
Oh and Rome Total War, also been a long time, maybe someone could confirm if you can play through it with mouse only. -
Since diablo-like games and RTSs have been suggested I'll try to suggest something a little different.
Neverwinter Nights/Baldur's Gate games might work for him. It can be paused anytime (mid combat) with a press of the spacebar I do believe (I think there is also an on screen button to pause it).
For an online experience I suggest Guild Wars. The RP mode (non PvP) battle is slower paced and there are options to change interface size to suit his needs. The whole game can be played using only a mouse. Guild Wars also provides a menu where everything can be accessed.
