How do I know when a patch comes out for [insert game here]?
How do I know the patch order? For CoD4, don't you have to install certain patches in order, for example? How do I check the latest version of Devil May Cry 4, for example.

All the patches that I've ever come across, even the huge one from Far Cry (1.4, 400MB), have been cumulative. That is, the latest patch includes all previous fixes from older patches as well as the fixes for the latest patch.
The download page should also have a notice somewhere stating whether or not the patch is cumulative; if they are not cumulative, I imagine that you'd have to install them in order. -
The patches will be labeled. Like for COD4, there is a 1.6 patch that will patch any version to 1.6. Then there is a 1.6 to 1.7 patch, so you have to install 1.6 first before it will let you install 1.7.
As for knowing if it's patched, somewhere on the title screen there will usually be a version number. -
Guess you don't remember the BF2 days, before they did those "Cumulative" patches.

Big game websites tend to announce the latest game patches and the order to install them in, best source is the official developer website. Check either of them periodically and that's it.
