Is there a way to change out the Graphics card in a Dell Inspiron 17R?
It says "Standard IntelĀ® HD Graphics
Optional ATI Mobility RadeonTM HD 5470 1GB"

Does this mean that I can change the graphics card to a better one?

Yup, the joker is right. Dell makes two versions of the 17R, one for each GPU option. You cannot change the GPU without changing the entire motherboard.

Nope. Dell does not make anything like that anymore. Very few notebooks can have their GPUs upgraded, and even then chances are slim an upgrade will ever make itself available.

A good rule of thumb is to not cheap out on the gpu you need/desire when it comes time to buy the notebook, because you will be stuck with it.
