Which do you use, and why?

    Or maybe balanced?

I don't use the Nvidia control panel. I set graphics quality on a per-game basis. Only time I need to use it is to force Antialiasing.

Driver level settings are only for things not changable in-game, like AA/AF for old games. Also, balanced/ performance just make everything look horrible, i'd rather use 640x480 and low settings.

    @ Spoodo

Can't help you on that. But try downloading NVTray to fix your customization woes. You can download it here: http://downloads.guru3d.com/download.php?det=1264

Anyway, I don't really see the difference when I set it to performance or quality. Mine's set to performance, but I also tweak some of the other options like setting Trilinear Optimization [on], Anisotropic Filtering [application], etc.

Did you try right clicking on your desktop and seeing if "Nvidia Control Panel" shows up?

Most recent tests, reviews, and tweak guides show that you usually don't take much (if any) performance hit using Quality options.
