Do you have v-sync on , also can be more specific on what settings you are running?

you're getting about half the fps that you should be getting

First thing i would do is to reset the setting in the nvidia panel to default settings. Then make sure that this game is set to use "performance gpu"

What driver version are you running?

You should get at least 60 on the average. My 580m usually is around 60-70.

go into Diablo 3 settings and disable v-sync, it might help i had something like this where my screen was 90 hz but it kept at a constant 45 hz, it might be the issue.

In D3, under Options->Video, what do you have "Max Foreground FPS" set at? Perhaps this got cranked down by mistake? Try turning it up to 200 and see what happens.

Check and make sure that your laptop power plan in Windows is set to high performance and not balanced. Hopefully this is causing the problem and it can often be overlooked. Let us know.
