If you've done all of what king has suggested then the only thing really left is to do a complete OS re-install following the exact steps king gave because I'm running on the latest Intel and WHQL getting anywhere from high 30's at the lowest up well past 50 depending on the situation,

-High performance profile
-Running on high performance GPU
-13.4 Cap 1
Very uncomfortable to play.
I'm running off clean installed OS which I installed yesterday. My steps are:
1)Chipset drivers.
2)Restart
3)Intel GPU drivers
4)Restart
5)AMD drivers
6)2 restarts just to make sure
Do you have any idea how boring that actions are and how much time does it take? -

From my experience, it's impossible to get this game running well on the 7970m. It's a horribly optimized PC port that's extremely CPU intensive, and like other old DX9 games it just doesn't run well at all with Enduro.

Mine is basic GTA IV just driving around and stirring up large amounts of trouble, keep in mind this game is heavily cpu bound and still runs comparatively poorly on my i7-3770K clocked at 4.5. I honestly don't know why you're having issues even after re-installing, I tried to find other posts/videos with similar problems but nothing came up with results as bad as yours. The only other thing I can think of is that your card is going/gone bad, how does it perform in other games?

Vahlen, you obviously have no idea what you're talking about. My i7-3610QM is plenty for 5 year old GTA 4. Poor GPU design on the other hand is not. Take a look at this screenshot made by me on the highest settings.
GPU usage 46% - under-performing.
FPS - 21 - slow
CPU usage 37% - So, CPU can handle 3 instances of GTA 4 running simultaneously without being a bottleneck (at least at that scene).

I played on friends Alienware M17x R4 with i7-3610QM and GTX 680M and got 40-50 FPS on the highest settings, no micro-stuttering, smooth gameplay. Isn't that flawless?

We all need to remember that if the 7970m wasn't crippled by Enduro it would outperform the 680m in quite a few games. The Nvidia card has a huge memory bandwidth bottleneck that becomes apparent with a number of games. This is coming from someone that has both cards. The Nvidia card may have better stability and driver performance atm, but from what I've seen running my 7970m in my brother's p150hm, that card beats the 680m in terms of raw performance. My 2c. -

I told you what I thought the possible problems and solutions were given the information you provided, if you don't like it fine, but to tell someone trying to help you that they have no idea what they're talking about is childish and rude. -
Enduro sucks hard. Doesn't matter which settings I change really or resolutions, the GPU usage never increases or decreases, stays around 50%. Lovely.
Luckily for me, I like the game fine with just SweetFX. Playing through first time right now, the $7.50 sale price for combo pack was nice. -

One thing to note about my old M17x's performance, I was running a constant overclock of +300/+600 at 1.075v, so my numbers aren't representative of a 680m on stock clocks. The M17x was a beast, it overclocked like crazy and peak temps never rose above 65C. I miss it.
