I start with Blur brought to you buy your friendly PC supporter Activision.
The video options for Blur: You can change resolution, the detail level, AA, and vSync. That's it, you're done.
Controls: No Wheel Support. None of the keys for keyboard or controller are remappable. Yes that's right. You can't change any keys on controller or keyboard and no wheel support. And the presets are well, terrible.

Worst for me was Saints Row 2, Call of duty: blackops.
-
Bad ports for me would be Saints Row 2 as well, and GTA IV.
Bionic Commando is pretty bad too, can't change any graphic settings. -
ghost busters.....no co-op...#@%@#^#@%...also very limited adjusted graphics
-
never tried blur on the console. Seems to run perfect on the pc.

Alright so just to be a clear, no adjustable graphics make the game a bad pc port? Even if the extras are lets say pre-emplimented, if you cant select it in a menu this concludes a bad port?
So all a dev has to do is give you a drop box menu for a list of lets say 8 options and have customizable keys (which I agree should be the norm by now), you'd all be happy? -
having adjustable graphics give people with lower range systems a chance to run it. -
So then what if they built for one set of hardware, so that everyone could play. its low spec'd so that everyone will have the same experience, the way the developers intended the game to be played. How bout ps3 and 360 spec'd , its low settings to today standards, but now the argument is the top end gamer misses out right. But what if there isnt alot of people spending 600 dollars on a video card, so its strictly an economical thing.
Also with one setting, the game can be optimised to the best performance on that one setting, -
worst console port... GTA IV , metro 2033... can't think of any else...

Star Wars the Force Unleashed!!!!!
Where to start, 30fps caps, can't adjust anything but resolution. Loading screen at every menu and that includes option and its sub menu. So annoying having to wait few seconds just to go from pause then to controls or changing the color of your lightsaber. And the massive amount of in game bugs. -
+1 for:
the first force unleashed, possibly the worst port by far
GTA IV
Black Ops (hopefully it gets patched) -
Crysis and Starcraft 2 are two games where you can adjust a lot settings for great performance and looking good. -
This is a bit old: Resident Evil 4... without mouse support!!

Mouse acceleration in Assassin's Creed 2 makes it a horrible experience to play with keyboard and mouse. Ubisoft knows this but wants to stifle it hehe.
Mouse Acceleration in PC version of AC2 - Topic Powered by Social Strata -
Star Wars: The Force Unleashed series wins by a long shot. I wouldn't be surprised if it they spent less than 30 minutes porting that game over.
