Lots of different things; one of the few times it was true, focused, killing rage though was when Sephiroth kills Aeris at the end of disc 1 in FFVII (not a spoiler by now,..)
Usually I just get angry at a bug, or the AI of teammates in an FPS being absolutely useless knuckleheads (in what modern FPS aren't they?). In FF7 it catapulted me into making killing Sephiroth my one focus. No idea why, that's just the way it was. *shrug* I also get really aggravated when I hit buttons and the game doesn't respond (or it's still stuck in the animations from the prior move). That one really bothers me in God of War.
I just realized both games I named are on consoles...bleh. It's hard to come up with specific examples atm, but retcons on new games that completely modify the story of the original get me angry. Random stuff like that... -
At least one controller has flown across the room due to Mario Kart, but PC gaming I tend to keep my cool better
I'm with JaiaV though... non-responsive control gets to me. I've gotten really pissed at Angry Birds because the damn egg-dropping birds won't drop when I tap and end up crashing into a wall instead.

Lately it's been Bad Company 2 and hit detection. Unload everything I got into someone and they keep coming at me like they're the terminator, take out their pistol and one-shot kill me. I even tend to yell at my kids when that happens, lol. It puts me in a bad mood. Then I get a grip and move on with reality.

A few times while raiding in WoW due to people committing the same errors and mistakes over and over again. Yogg pre-nerf in particular, but all those difficulties made downing him feel so much more satisfying.

Tons of games in MW2, MAG gets me from time to time, having a crappy team who team kills in L4D get me, trying to figure out a puzzle in Zelda OoT for the first time ( I was like 9 or 10 playing that), Raiden arcade version..... Man I love games soo much but im realizing how angry it makes me feel sometimes, oh and this list is never ending, missions in GTA get a mention too.

I mostly rage when people cheat online.
I raged when I couldn't get past the chocolage island stages in Mario World SNES.
I FREAKING raged at NES Back to the future dish-throwing stage...
I Raged @ Perfect Dark 64 when I couldn't ever get to Rank 1, and was stuck at Rank 4: Elite
I raged @ Jurassic Park Park Builder on GBA when I couldn't get a single soul to stay at the hotels.
I always rage @ Mario Kart when I use a mushroom right when someone bumps me and I'm wasting all that juice against a freakin' wall.
I raged in Dragon Age, when I befriended Morrigan, did the dance with no pants, and she only wanted to 'be friends'.

1. You know you're the Alpha male in a game, a newbie suddenly appears with his 5-day old Clan members, everyone speaking in French or whatever European language comes to mind.
You're CHILLEN with your buddies talking about last day.
Newbies spamming the channel, killing everyone...
Teammates make you fight 1 vs 1 against the newb's leader, you know its gonna be too dang easy so you take it down a notch.
Chat, taunt, laugh and fight against the leader and you know he's down to like 1 health
You suddenly want to coup-de-grace your inferior opponent with the weakest move/weapon or jumping monkey maeuver you can...
Lag.
FREAKING Lag.

Any game that has Quick Save/Quick Load button.
You obsess over the Quick save.
The moment you decide to go Indiana Jones and get FAR ahead in the game is when you suddenly forget about Quicksave and DAT WINDOW KEY screws everything up.
Also,
Those dang keyboards that have a Sleep and/or Shutdown button under the delete, end, pagedn key...

Usually when I reach this point in a game I just stop playing it. Any game that frustrates me that much is not worth playing because it isn't fun any more. Examples would be GRAW2 or Hitman Blood Money.
Usually the most frustrating games are the ones that require extensive strategic planning, but the design of the game cripples your plans. -
I've had a lot in my gaming life. Probably my most memorable was playing Xenosaga 2 and trying to figure Albedo's (the white haired guy) weaknesses around 1/3 of the game. Really ... frustrating.

STACKED TEAMS! I swear if there's one thing I HATE about Bad Company 2 is that every server I join there is a stacked team. Then after a few rounds my team becomes stacked (tbh, it's a nice change of pace). And of course all the "wow that was easy, you guys suck"...RAWR *throws chair*
Oh yeah, and snipers in every FPS sitting back in a spawn point not hitting anything and taking up space. -
The heavy breather. Seriously, I swear there is one in every game.
The commando noobs are the worst. They run around the map at the speed of light and somehow teleport their knife lunge. -

I would also have to say BFBC2 hit detection. Today I shot an enemy twice from about 3 feet away with a pump action shotgun (which is easily a one hit kill at that range); neither hit was detected and he killed me. Also, half of the time when I am sniping a moving enemy I will see the bullet hit, but get no kill because the hit was not detected.

Today at league of legends dropped from 1915 elo to 1850 elo straight losing streak with a bunch of people who just feed bottom and complain about their own team! felt like fliping my desk over but then i thought my laptop would feel the punishment and didnt wanna do that lol

I don't rage very much, but when I do it's pretty nasty.. Usually it ends with me yelling profanities, slapping my monitor, smashing my fist on the table, or doing something similar
Counter Strike Source: I got it a few years back when it was like the most popular FPS game on Steam and people were raving about it. I join a server and I begin my first match. Buy gun, kevlar, nades, and I run out to get some kills. Boom I get headshotted. Okay I wait for 3 min to respawn... I respawn and rebuy weapons again. Oh look my teammate just through a flash and now I'm blind. Oh look I'm dead. Okay lemme wait another 3 minutes... Needless to say I uninstalled CSS THREE TIMES out of rage. The fourth time I decided to give it a shot and I eventually got the hang of it from there.
L4D2: I feel as though this game requires way to much common sense for people. So we're moving through Dead Center and I get pounced on by a hunter. Two of my teammates are a distance behind me fighting other zombies and I'm left with one teammate who is like 5 steps away from me. He LOOKS AT ME while I'm getting torn up and instead of shooting the damn thing he runs off to pick the Molotov.. He then throws the molotov on me and the hunter. And the hunter obviously didn't die on impact so he had enough time to rip my face off. I mean dude YOU HAVE A GUN. Shoot it.. Now I don't play l4d2 unless I know my teammates have graduated from Elementary School and are capable of left clicking. -
For me, it has to be certain situations in WoW. Like I just started playing elemental shaman in pvp, and the amount of faceroll NOOB players (for christs sakes, keyboard turning clickers) that kick my because of class balance...
Gives me nerd rage headaches (if anyone is familiar with those) -

No rage from me, but sometimes I do get upset a tad when playing 2v2 random in SC2 and my team member does something incredibly dumb like try some cheese tactic and just totally screws us over.
FPS games tend to get me on edge more, but I have not been playing them of the late, usually I can only blame myself for the upsetting moments in fps though, but it sure does suck when the same guy kills you like 5 times in a row, especially if he is a wookie camping asshat. -

I get pretty pissed in Crysis Wars sometimes. I'm extremely dominant in that game, and get accused of cheating a lot. It pisses me off that some players can't accept the skill set and tactical superiority and in their minds have to chock up my ownage to "cheating". It's insulting to me and doesn't give credit where it is due.
That kind of thing sends me into nerd psychosis. -
2v2 SC2 game with some friends. I got rushed+totally wiped out. Rage quit, no gg and just ignored calls/texts from the guy who rushed me for the rest of the day. Haven't touched SC2 since then. This was approximately 3 months ago XD
P.S. I'm more of a Zerg player (ever since BroodWar), so you know what I mean -

Physical rage. Ahh...how I miss those days.
The CD-ROM cover on my PSX broke in two due to my inability to see where my save game slot was, and overwritten it with another. The PSX probably cried in pain thereafter...
But then again, because of that incident, the PS2 came trotting along in my room.
Now, I rather shut up and play than doing the tango with my precious electronics.
