I've only started experiencing this recently, but now when I play source, my fps is 50 which is normal. In-game however it gets very jumper, about twice a second it kinda lags for a few miliseconds. it's really hard to explain, but it's driving me nuts, my head hurts and i cant get any headshots because of this. it's not lag, my ping was 20 and I got it on every server i went to. i also thought it could be caching stuff, but how come it only started happening now. i only have steam and source open - nothing else ram hogging.

    help!

Is it every time you play now? I've had this issue before, and I know how annoying it is. It happens very rarely for me, usually after I leave my machine on for a long time. A restart usually fixes it. If that doesn't do it for you, then try reinstalling the game.
