Perhaps heat is the main cause for video card failures? Are there other causes in normal use?

Over and undervolting can both damage the hardware as well.

Are we not counting Nvidia's infamous defective substrate material as well?
Poorly applied TIM and poorly designed cooling system will contribute. Also if the package material in the die isn't design properly, you get Nvidia chip round 2. -
But I don't see how hardware can be damaged when undervolted. Undervolting and underclocking if anything helps to increase the lifespan of computer componentssince both techniques lead to cooler operating components

But another thing that can cause video card failure is obviously a mechanical damage. My ATI on my previous laptop died because of the cheap Fujitsu-Siemens construction that allowed the whole MB and all components to bend ...
