    Tablet PC for Medical Records

    Pardon my ignorance, but I've been searching for quite a while and I am not getting anywhere.

    I would like to turn my office as paperless as possible, I need to record patient data electronically and add attachments to the records then store these electronically. Retrieve them easily and update or add info to their patient record/file during subsequent visits.

    The ultimate would be to actually have the form needed displayed on a computer screen and hand write, (fill in the appointment data) actually on the screen. Then be able to attach (scan) any hand written patient information forms to the file. Then save it to a database.

    Do such hardware/software systems exist? Are they reliable and secure?
