I'm not going to post up the details. Printing up a form, filling it out, cutting out and sending in the UPC isn't very hard. You guys can read it and figure it out yourself. I have already given the most relevant information as far as I can tell. If you did purchase from the above sources, count yourself out. Anyone else, feel free to try it for $50 less postage. -
