I understand its Samsung or Hynix on a T61. Does anyone know it has a heat spreader? If so, necessary to get similar memory with heat spreader? Intuitive answer is yes but chime in. Tks.

My hunch tells me that there is no heat spreader, and that you would want to get sticks that do not have heat spreaders. But that's my experience with other brands talking...

No laptops as far as I know come with memory that have heat spreaders on them. The ThinkPad definitely does not have them. I would suggest not to buy memory with heat spreaders on them because of the cramped memory compartment.
