I called Lenovo to find out why I don't have the UltraNav tab in mouse settings of my x200s. The rep took some minutes to look up the info, then came back and said she didn't see ultranav driver for X200s, only trackpoint driver. She said maybe this particular model just does not have UltraNav feature. Could someone check their X200s to see if it's like mine, no UltraNav? Thanks!

yea i believe there is no ultranav for the machines without a touchpad. someone please correct me if i'm wrong.

alphakenny1 is correct. ultranav = trackpad + trackpoint. it has absolutely nothing to do with the center trackpoint scroll button found between the left and right mouse buttons.
all current thinkpad systems have the center navigation button. if you are missing the tab to change trackpoint settings then i suggest reinstalling the trackpoint driver. -

below are two pics of what you'd see on a trackpoint-only system (like the X200s) and what you'd see on a system with ultranav (ie: USB keyboard or full-size thinkpads). the "lenovo" tab you see is there because i have a thinkpad bluetooth mouse installed for use with both my D10 and X61s.
if you don't see a trackpoint tab then you'll need to go to the X200s driver matrix page and download the appropriate driver.
Lenovo said No UltraNav for x200s, could someone check yours?
