I tried a Skype call using my T420, but the mic seems to pick up a lot of background and computer noise, making it somewhat muffled.

is this quality normal for ThinkPad microphones? It seems worse than my T400's mic.

Mine (W520 duel mic) is bad when using Ventrilo, apparently its driver issues but they only seem to support stuff like Skype and such which got fixed in the latest driver.

Did you try adjusting the settings (Fn+F6)? Start with the option to select one voice or multiple voices.
