I recently bought a secondhand adapter (from a seller on another thinkpad forum with plenty of positive feedback so I don't have reason to believe it's fake) and I just noticed some noises I hear that I assumed came from my laptop's hard drive are actually electrical noises coming from inside the adapter itself. When I scroll using the middle button and the trackpoint, there is a very distinct noise (similar to the noise made from accessing a large file on my hard drive, albeit louder) that I can hear from 4-5 feet away. Is this normal and should I worry about the power supply exploding?

That noise is, unfortunately, normal for some PSUs. Even a brandnew 90W PSU that came with a T410 made exactly the noise that you describe.

I have a 90W adapter but do not hear any noise from it unless my ear is right on the adapter casing. So you might want to ask the seller if that's a normal condition, if he think that's abnormal arrange a replacement.
4-5 feet would be a pretty loud electrical noise considering for a supposedly silent PSU brick.
