My volume keys only work if explorer.exe (desktop, taskbar, explorer window, etc.) has focus...

If another program has focus pushing the volume button has no effect.

Does this happen for anyone else?

UPDATE: More specifically... the volume up/down and mute don't work, but the mic mute does work.

If you press the button, does it bring up the small screen of volume bar?
