That is a great review though. I was debating on what type of card to get in my upcoming laptop purchase. This secures my choice

Yes, I kind of found it strange that they said the cards couldn't be bought on their own. Maybe they couldn't find them so readily available, but then again... They're on ebay and on amazon, as well as quiet a few other retailers... Just as you stated.
Also maybe they could have run the test more times. But I think the results are pretty accurate from what I have read in this forum and other sources. -
I have been told that also.
I was also told that I hadn't bought a genuine Killer wireless card if I got it on its own. Funny it calls itself in my bios a killer card and also in the registry -
How actually do you connect the wireless card to your notebook?
Do you need a pci -> usb connector ? -
You need a mini-PCI Express card that you use instead of the stock one.

Afaik, nope. You'd need a mini pci-e to usb adapter and that kinda defeats the purpose of mini pci-e in the first place. Either you buy a mini pci-e adapter to install in your notebook or you buy a USB adapter. I've seen pci-e to mpci-e adapters to allow running a laptop card in a desktop, but never mini pci-e to usb.
