Is it possible for a usb wireless dongle plugged into a desktop to act as an access point without being logged into XP home?
also, if you put a router inside of a computer case, how much do you think it would drop the gain?

A metal case will have a dramatic impact on the router if the antennas are inside. If the case is empty with the router inside it will work as a shield. It will impact the capability of clients to connect, mainly due to the lower power output. If the case is actually a working pc it will impact it more due to RF noise. My guess is a 40-50% hit.
If you mount the antennas out side the case it will have little impact as long as the antenna leads are less than 6'. Any length greater than that will cause a power drop.
As far as using a USB Client as a AP depends upon how it is setup. If you have it setup in adhoc mode the connections can be established. But most Client USB's do not have the capability of working as a AP, if the pc is use MS windows. The Linux OS does allow you to set them up as a AP. -
