    That's classic tearing. Tried v-sync?

Double buffer, triple buffer and/or limit fps? Also ... maybe run an fps log with Afterburner to see what is taking place, exactly. Instead of limiting gpu, might also adjust panel's refresh rate ... a bit faster, preferably.

Yes, but sadly enough, your system is Optimus (didn't count on that). Anyway, image to display passes through Intel HD. So refresh rate will be limited to the iGPU, even when running dGPU. Hmm, what panel do you have? Run MonInfo and use ' File -> Create INF' on the proper display.

Inf is very useful, actually. CMN15B1 isn't enough to narrow it down; could be any of these five panels. Also need it if you want to run it at higher refresh rate; a hassle to force this on gpu switching system (either Enduro or Optimus) and make it stick.

Did Intel HD work? A lot easier, but a 75-90Hz panel has its own merit, whether it solves tearing or not.
