    I've never had that happen. Will be starting new game+ when I finish a few other games I just got (I hate, you steam sales!). This was all before the last patch. Maybe it's a keyboard-specific issue? I played with my PS3 controller.

    May be I guess. But you should have faced the phase walker issues. I have read thousands of post about it. I am not able to continue the game because of it. The reaper form of course activates well using a controller I guess. But not sure about the phase walker bug.
