    I have a system I am trying to repair where the desktop does not come up.
    I opened task manager and manually started Explorer.exe but it was killed after bringing up the desktop for an instant. I have run Combofix and other malware removal tools - any ideas?

    Are you running your virus/malware scans in safe mode? IF not I would to that first.
    If you are sure that it is not a virus or rootkit, One trick I have used in the past was to reload service pack 3 for XP .

    Re-boot your computer in safe-mode. If it doesn't work, reinstall operating system and install using BIOS.
