Currently I haven't bought, downloaded, and installed BF4 yet. If I make a full image backup using macrium reflect without the game installed on the hard drive. In the future, if the game is installed and I wanted to upgrade the hard drive using the image above, will I have to re-purchase the game and reinstall it again? Or is the purchased game tied to the origin account?

The game will be tied to your Origin account, so all you have to do is reinstall Origin, download the game and reinstall it.
