Wow i'm stumped on this one. It's a Toshiba Satellite A355-S6998E. It randomly enters sleep mode by itself with NO user interaction. I disabled sleep mode in the power settings and it still does it. I even reinstalled the OS, no luck. I do have a Microsoft Explorer Wireless mouse hooked
to the laptop and that's it for external devices.

    Specs are:
    Satellite A355-S6998E
    Intel® Core™2 Duo Processor P7350
    Windows 8 Pro (64-bit)
    4GB PC6400 DDR2 800MHz SDRAM
    320GB HDD (5400rpm, Serial ATA)
    DVD-SuperMulti (+/-R double layer) with Labelflash™ drive
    Intel® Wi-Fi Link 5100AGN (802.11a/g/n)
    No Bluetooth (No Antenna)
    86 key US keyboard
CD/DVD Buttons (Play/Pause, Stop, Prev Track, Next Track)
    Media button
    Mute button
    Touch pad pointing device
    Li-Ion (4000mAh, 6-Cell)
1-eSATA/USB (2.0) combo port with Sleep and Charge
3-USB (2.0) with Sleep and Charge

My guess here is the computer is bored, you just need to spice things up a bit...... :)

So you said sleep is disabled, so then is it hibernating now or still sleep to ram?

It did come with Vista, never used Vista long enough to find out. It was Windows 8 little brother IMO. :p

I do think it's OS related, so I installed Windows 7 multi-boot to find out what might be causing this issue.
Ha, yeah it could be due to boredom, never thought of it that way. :)

I started from scratch and reinstalled Windows 8 in a dual boot mode. Windows 7 is fine no sleep issues whatsoever. So far Windows 8 is stable with no random sleep entries. What I did this time is I only installed Windows 8 SECURITY-CRITICAL updates. No MS drivers or improvement updates were added.

One thing I noticed immediately is that when I now perform a disk cleanup to remove Windows 8 update entries the hard drive no longer sits for several hours thrashing trying to perform a cleanup and the CPU is not pegged @ 99%. This has happened on several laptops where all Windows 8 updates both critical and non-critical were installed..
