This has actually been happening for quite a while but didn't really pay attention to it until recently when I decided to look for a fix. I tried searching the forums and couldn't find any similar problem.
So basically, when I highlight an application (.exe file), the icon beside "Open" is missing. This is the only location where the icon is missing.
I already tried clearing the icon cache and restarting with no luck. Also, System Restore is a no-go.

Maybe it's to do with the fact that it's an obscure executable. Same thing happens on mine.

EDIT: I actually found the fix for it:

I don't know if that's the proper icon to be displayed, so if anyone has any other shortcuts to other icons that can be changed in the registry or if someone could post a screenshot on how it looks on their laptop without the problem.
EDIT 2: If I remember correctly, the icon beside "Open" is supposed to be the same as the one that is circled in the picture:
Either way, the solution found in the article is better than nothing. I just wanted to see what other people have done in terms of this problem. -
Oh that's weird. Now the icon is completely different to the Windows default...
