Personally, I've used CCleaner for a while now with no problems. But now I'm considering using a Registry Defragger, which sounds like it works similarly to a Disk Defragmenter; which doesn't delete anything, so am I able to assume it's safe?
The defragger I'm considering using is called "Free Registry Defrag" by 'IExpert', it was mentioned in the latest Computer Shopper as a free utility to optomise your PC. It seems that if either of these programs will cause a problem it would be CCleaner as it actually deletes files (though it allows you to back-up the registry first).
Edit: I just tried the Registry Defrag program on my PC (it's my laptop I don't want to risk) and it seems a good basic program that just does it's job. It said my registry size was 21mb and it reduced the registry by 2-3mb...I can't really see 2-3mb making much of a difference. The PC still feels fine; it was fast before anyway so it's hard to tell if it is any faster...

If your using XP you could try the JkDefrag Gui, which when enabled has a registry defrag app and also you can enable boot defragging, so the boot files are always tidy. I use it on my work notebook and it's always nice and quick. It's also free and comes with the Defrag program which is great.

I use Auslogics Registry Defrag and never have any problem, but to be honest I do not see any noticeable improvement

The built in Registry compacter in JKDefragGui is NTREGOPT which is safe. Works pretty well too.
