    the only one i've had much experience with is DVDFab i'm not sure about fast but it has worked with every disk i have tried

    of course i just use it to back up the entire disk not neccesarily rip to some format

    if ripping to different formats etc you can take a look at handbrake and/or super

    I like for DVDs because it has an option to rip the ISO image. My WDTV player plays DVD ISO files with full DVD menus. It doesn't rip everything, but it works on the vast majority of DVDs. When doesn't work, I fall back on, creating the VIDEO_TS file under a directory named for the DVD.

    I have not found a good BR solution.
