Which is the best as far a video quality is concerned? I'm not concerned about file size, just quality. Is there a way to get MPEG 4 video's to show previews in folders?

I would say MPEG4 using the XVID codec provides the best possible quality to bitrate ratio.

Mpeg 4, just be sure to give it a nice codec when encoding. It depends on what you're encoding though.
