Pretty sure your system is throttling. Could you download GPU-Z and run it in the background and take screenshots of the logger?

You could also force your clocks to stay at a certain speed.
    1) Download AMD GPU clock tool and install it.
AMD GPU Clock Tool v0.9.26.0 For HD 5870 download from Guru3D.com
    2) Disable powerplay in CCC
    3) Set you clocks to 625/1000 (default maximum) or you can go higher to 650/1050 or 700/1100 but higher than that will cause overheating issues.
    4) Try running your game after that. It should remove the lag.

This helped me out. So it will probably help you out.
Also make sure you are using the lastest BIOS (A08). This has solved issues with throttling.

I doubt its throttling. GTA4 Pc has a rep for not working completely well on Pc. If all your settings are right ad it still dosnt work then the only thing you can do is get a i7 920xm, which people claim helps alot. Other than that theres nothing to do.

Yup I disabled powerplay so that the clocks dont go down during game play.
Also what drivers are you using? I am currently using stock ATI 10.8 driver. Somehow the dell drivers were giving me BSOD even after a reformat.
Also make sure your BIOS version is A08. It has massive improvements in performance. Plus with A08, there is a 99% chance that you don't have to perform all those steps.

I used to get around 37 FPS with 720qm/5850 combo.
Now I get around 40 FPS with 920xm/5850 combo.
